Understanding The Risks Of At Home Ketamine Injection

You may have recently heard of at-home ketamine injections: kits delivered to your home that allow you to give yourself a shot of ketamine. Considered the “next phase” of self-administered ketamine therapy, these injections are touted as completely safe, more effective, and better tolerated than oral ketamine. But self-injecting ketamine is inherently risky, as is at-home ketamine treatment in general. Please read our blog to learn about the risks of ketamine injection without direct medical supervision before you decide to try this method.

If you're dealing with anxiety, depression, PTSD, or a treatment plan that just isn’t working — we solve with ketamine therapy and behavioral health care built around you.

Why Would Someone Inject Ketamine at Home?

Traditionally, at-home ketamine therapy has been administered through oral lozenges or sublingual tablets (placed under the tongue), which are easier to take and less invasive. The at home ketamine injection kits are administered subcutaneously (into the abdomen), which offers a few advantages to oral routes:

  • Faster Onset: Subcutaneous injections produce effects more quickly than oral ketamine, which takes longer to be absorbed and metabolized. They are not nearly as rapid as intramuscular (IM) injections, however (see below).
  • Higher Bioavailability: Injected ketamine has higher bioavailability: it bypasses the digestive system, which can lead to a stronger and more predictable effect compared to oral administration.
  • Cost and Accessibility: Some people seek out at-home injections to avoid the cost and time commitment of in-clinic treatments, believing they can self-administer safely without professional oversight..
  • Better Comfort And Consistency: Oral ketamine is meant to dissolve slowly, but many patients accidentally swallow it. This means it has to go through the digestive system instead of being absorbed through the mouth directly into the bloodstream. Effects may present much later or not at all.

How Does An At-Home Ketamine Injection Work?

Presumably, individuals prescribed at-home ketamine injections will receive vials of ketamine pre-dosed by weight, along with sterile syringes to draw up the medication and alcohol wipes to clean the injection site.

When it is time for a ketamine injection, the patient connects with a healthcare provider virtually, who then guides the patient through the administration process and the dissociative after-effects. As with oral at-home ketamine, a friend or family member must be present as well for safety.

Risks and Side Effects Of At-Home Ketamine Injection

Self-administering ketamine injections at home carries significant dangers, including:

  • Potential For Incorrect Dose: Ketamine dosing is typically based on a person’s weight in kilograms. Should a patient give inaccurate weight information or miscalculate from pounds to kilos, they could receive a dose that is too high or too low. Either case is dangerous: an ineffective dose could have disastrous consequences for someone with acute suicidal ideation, while an overdose could lead to dangerous spikes in blood pressure, respiratory distress, and more.
  • Cardiovascular Effects: Ketamine can cause elevated heart rate and blood pressure, which may be dangerous for those with underlying conditions such as hypertension or heart disease. In a clinic setting, patients will have vitals continually monitored during the session and for up to 2 hours afterward. There is no such after-care provided with at-home ketamine injection.
  • Injection Complications: Improper technique can lead to serious bruising, infections, abscesses, or accidental intravenous injection, which can result in dangerously rapid effects and unpredictable reactions.
  • Potential For Injury: Ketamine is a dissociative drug. You are meant to have a dreamlike experience during your session, which means you may also have auditory and/or visual hallucinations. You may feel the urge to move around, which could lead to falls or collisions with stationary objects. In-person guidance ensures you remain in a safe, comfortable position without the ability to endanger yourself.
  • Psychological Distress: Some users experience severe anxiety, frightening hallucinations, or emotional distress while taking ketamine. Without professional support to guide them through the experience, they may develop a negative association with the drug and refuse to continue treatment.
  • Lack of Emergency Support: If a serious reaction occurs, such as a severe panic attack, loss of consciousness, or an unexpected adverse effect, there is no immediate medical intervention available.

Why Medical Supervision is Critical For Ketamine Injection

You want to get the most out of your ketamine therapy, and this should entail working with a licensed medical professional. While we understand the convenience and affordability of at-home ketamine injection is tempting, the risks far outweigh the benefits. Working with a healthcare provider ensures the following:

  • Proper Dosing & Administration: Medical professionals ensure the correct dosage, route of administration, and monitoring for adverse reactions, reducing the risks associated with improper self-administration.
  • Rapid Onset Of Effects & Symptom Relief: Injections administered at a clinic are given into the muscle, typically in one arm. This route has a bioavailability of 93%, which means the effects(and symptom relief)  will occur much more rapidly. This is ideal for patients in immediate distress, such as those with acute suicidal ideation or severe PTSD.
  • Immediate Medical Intervention: If an emergency occurs, trained staff can respond quickly with necessary medical care, including oxygen support or medications to counteract adverse effects.
  • Guided Experience: Licensed professionals can help patients navigate the dissociative state, offering reassurance and a structured therapeutic approach to maximize benefits while minimizing distress.
  • Legal and Ethical Oversight: Receiving ketamine under professional supervision ensures compliance with medical guidelines, which protects both patients and providers while preventing potential legal complications associated with self-administration.
  • Follow-Up Care: Professional ketamine clinics incorporate therapy and follow-up sessions to ensure long-term effectiveness. You are taught how to process what you saw and heard and how it made you feel. You also learn how to incorporate the insights you gained from your sessions into your daily life. This is known as “integration,” and is a critical part of ketamine assisted psychotherapy.

At home ketamine integration is done predominantly through online group forums. While better than nothing, these platforms lack the individualized approach that is key to long-term success with ketamine therapy.
