Your annual well woman exam will generally include three parts: vital signs, a breast exam, and a pelvic exam. Depending on your age and medical history, you may be referred for a mammogram, and your pelvic exam may include a Pap smear. Here's what to expect at each step.

Vital Signs

These quick, non-invasive checks provide a snapshot of your overall health and help identify early signs of potential problems.

Blood Pressure
Heart Rate (Pulse)
Respiratory Rate
Temperature
Oxygen Saturation (SpO2)
Weight and Height

If a vital sign falls outside the normal range, it doesn't always mean something serious is wrong — but it does signal a need for further evaluation, which may include a recheck, a medical history review, further testing, or, in critical cases, urgent treatment.

Breast Exam

Breast exams are an important part of preventive healthcare, helping detect changes or abnormalities early, when treatment is most effective.

What The Exam Looks For

Lumps or thickening in the breast or underarm
Changes in breast size, shape, or symmetry
Skin dimpling, puckering, or redness
Nipple discharge or changes
Tenderness or swelling

What's The Process Like?

The exam typically includes a visual inspection followed by a manual examination, done in a circular, up-and-down, or wedge pattern to check for any lumps or irregularities. It usually takes just a few minutes.

If your provider finds something, this does not mean you have breast cancer — most lumps turn out to be benign. Follow-up may include diagnostic imaging, a specialist referral, or monitoring over time.

Annual Pelvic Exam & Pap Smear

A routine part of preventive care, assessing the health of your reproductive system — the vulva, vagina, cervix, uterus, fallopian tubes, and ovaries.

External Exam

A visual inspection of the external genital area (vulva) for signs of irritation, cysts, or unusual discharge.

Speculum Exam

A small, smooth instrument is gently inserted into the vagina to allow the provider to view the cervix. This is when a Pap smear may be performed.

Bimanual Exam

The provider checks the size, shape, and position of the uterus and ovaries.

Does It Include A Pap Smear?

Often, but not always. A Pap smear screens for cervical cancer by collecting a small sample of cells from the cervix. General guidelines: ages 21–29, every 3 years; ages 30–65, every 3–5 years depending on HPV testing; after 65, may not be needed unless risk factors are present. Your provider will let you know what's right for you based on your health history and age.

Abnormal results are common and don't always mean cancer — they may indicate minor inflammation, an HPV infection, precancerous cells (which are treatable), or a benign condition like a cyst or fibroid. Your provider will explain the findings and next steps.
